Deze patiënt wordt behandeld voor pulmonale arteriële hypertensie met riociguat. De riociguat-tablet wordt driemaal daags ingenomen.
De dosering van de tablet kan variëren van 0,5 mg, 1 mg, 1,5 mg, 2 mg tot 2,5 mg. Ronde filmomhulde tablet.
Waarschuwing
Riociguat mag niet gebruikt worden in combinatie met een PDE5 remmer (sildenafil, tadalafil, vardenafil) en/of in combinatie met sterke CYP-remmers van meerdere routes en Pgp/BCRP-remmers zoals azoolantimycotica (bv. ketoconazol, itraconazol) of HIV-proteaseremmers (bv. ritonavir). Bij combinatie met een anticoagulans, extra zorgvuldig controleren op (long)-bloedingen.
Riociguat
Riociguat zorgt voor vaatverwijding in de long via de NO-pathway. Hierdoor verbetert de pulmonale vasculaire hemo-dynamiek en neemt het inspanningsvermogen toe.
Behandelingsrichtlijnen
Bij start van Riociguat wordt er gebruik gemaakt van een titratie-schema onder begeleiding van de behandelend arts. Na acht weken heeft de patiënt de riociguat-dosis bereikt die voor hem/haar de juiste is. Als de behandeling om wat voor reden dan ook drie dagen of langer wordt
afgebroken dan moet de behandeling weer met drie maal daags 1 mg worden opgestart en met het titratieschema worden voortgezet (onder begeleiding van de behandelend arts).
Bij onbegrepen verschijnselen kunt u de productbeschrijving van alle ingenomen medicijnen raadplegen om na te gaan of er sprake is van medicijninteractie, bijwerking of medicijneffect. Bij problemen direct contact opnemen met de behandelend longarts.
Bijwerkingen
Hypotensie, duizeligheid, hoofdpijn. Dyspepsie, diarree, misselijkheid, braken, perifeer oedeem.

Warning
Riociguat can not be used in combination with a PDE5-inhibitor (sildenafil, tadalafil vardenafil). Riociguat cannot be used in patients receiving strong cytochrome P450 (CYP) and P-gp/BCRP inhibitors such as azole antimycotics (for example, ketoconazole, itraconazole)
or HIV protease inhibitors (for example, ritonavir). Increased risk of (respiratory) bleeding in combination with anticoagulantia.
Riociguat
It leads to relaxation (vasodilitation) of the vessels in the lung through the NO-pathway. It will improve the pulmovascular hemodynamic and will leads to better exercise tolerance.
Treatment guidelines
The dose of Riociguat will be uptitrated in the first eight weeks of the treatment if the patient has no signs or symptoms of hypotension. Dose increase should be no sooner than two weeks apart. The dose can be increased to the highest tolerated dosage, up to a maximum of 2,5 mg
taken three times a day. If the treatment is discontinued voor more then three days then the patients should follow again the uptitration scheme. Often several medications of pulmonary hypertension are prescribed concomitantly. In case of problems refer to the product description of all medications to see if they result from medication interaction, side effects or medication effects. Immediately contact patient’s pulmonologist in case of problems.
Side effects
Hypotension, dizziness, headache, gasto-intestinal complaints, nausea, peripheral oedema.
