Macitentan
Een witte, ronde tablet. Het is een duale endotheline-receptorantagonist, die zowel aan receptor A als B bindt. Het geneesmiddel vermindert de pulmonale vaatweerstand bij patiënten met pulmonale hypertensie.
Waarschuwing
Macitentan mag niet gebruikt worden in combinatie met bosentan of ambrisentan. Voorzichtigheid is geboden bij gelijktijdige toediening met CYP3A4-remmers (zoals itraconazol,
ketoconazol, claritromycine, ritonavir). Raadpleeg de SMPC voor een compleet overzicht.
Behandelingsrichtlijnen
Er kunnen leverfunctiestoornissen en anemie optreden. Regelmatige controle van de leverfunctie en hemoglobinegehalte is daarom noodzakelijk. In geval van een verhoging van ASAT en ALAT groter dan 2 ULN dient de behandeling gestaakt te worden. Bij onbegrepen verschijnselen de productbeschrijving van alle medicijnen raadplegen om na te gaan of er sprake is van medicijninteractie, bijwerking of medicijneffect. Bij problemen direct contact opnemen met de behandelend longarts.
Bijwerkingen
Onder andere: nasofaryngitis, bronchitis, hoofdpijn, anemie, vochtretentie, hypotensie, urineweginfecties.

Macitentan
A white, round tablet. It is a dual endothelin antagonist binding to both receptor A and B. It has a proven effect on recovery off blood vessels in the lung, in patients with pulmonary hypertension.
Warning
Macitentan is not to be used in combination with ambrisentan, bosentan. Caution should be excercised when Macitentan is administered concomitantly with strong CYP3A4 inhibitors (e.g.

Treatment guidelines
Patients may develop abnormal liver function and anaemie. Regular checking of liver function and haemoglobin level is necessary. In case of AST or ALT greater than 2 ULN stop treatment. Often several medications of pulmonary hypertension are prescribed concomitantly. In case of problems refer to the product description of all medications to see if they result from
medication interaction, side effects or medication effects. Immediately contact patient’s pulmonologist in case of problems.
